PDQ Order Forms and Fees

On this web site we offer our service request forms in Adobe PDF files. If you are unable to open or print the request forms, please contact us (248-324-0099 or toll-free 866-324-0099) and we will fax or mail them to you. Completed request forms can be transmitted by e-mail (mitchmallin@qdroexpert.com), fax (248-324-1497) or by mail (Pension Division by QDRO, LLC, P.O. Box 2247, Farmington Hills, MI 48333-2247). The required fee must be paid by check and mailed.

Valuations: $400 fee for the first calculation and detailed report and a $125 fee for each additional benefit commencement age requested. 72 hour service is available for an additional $125 fee.
