We are no longer requiring prior authorization for new work assignments due to improvements in workflow and increases in capacity. Our standard request forms can once again be accessed directly on this web site by clicking on "Forms & Fees" above.

Please note that the following policies are still in effect:

1. We will not prepare QDROs for defined benefit plans where Fidelity is the plan’s QDRO reviewer due to continuing problems with Fidelity.

2. The parties retaining us must be represented by attorneys. Completed QDROs will be mailed to the attorneys for the parties retaining us.
